Abstract

Background: Hematological malignancies can involve the GIT along the disease course having bad prognosis. Trans-abdominal ultrasound is sensitive to assess bowel abnormalities.
aim: Detect the prevalence of GIT involvement in patients with hematological malignancies using trans-abdominal ultrasoundwith validation by endoscopy and to assess the reliability of trans-abdominal ultrasound as an initial diagnostic tool for early detection of GI affection.
patients and methods: Sixty patients with haematological malignancies were included in the study. All patients underwent abdominal US and US guided biopsy in selected cases. Patients with positive findings in stomach/bowel/pancreas underwent endoscopy and histopathology.
results: 18 patients out of 60 (30%) were found to have bowel involvement by trans-abdominal ultrasound. There were 9 dropouts (4 due to death and 5 lost follow up) and only 9 patients underwent endoscopy .Three patients (5%) were found to have blood malignancy infiltration. Factors associated with positive bowel findings in ultrasound are : treatment naïve, abdominal pain, diarrhea , fever and low platelet count (p value=0.016, 0.000, 0.021 and 0.010 respectively). Asymptomatic patients were statistically significant higher in negative population (p value=0.001).Ten patients died before the end of the study.The factors associated with mortality are abdominal pain, vomiting ,fever, high TLC, low platelet and low albumin levels (0.000,0.002 and 0.012 ,0.014,0.026 and 0.043 respectively).
conclusion: Ultrasound is a simple, safe, and sensitive tool for screening and early detection of GIT involvement in patients with hematological malignancies improving treatment options and prognosis
