Abstract

Abstract
Case control study included total 85 subjects divided into 3 selected groups: group I: 30 chronic haemodialysis patients with NAFLD, group II: 25 chronic haemodialysis patients without NAFLD, group III : 30 healthy volunteers as control group. Patients were recruited from the dialysis units of Ain Shams university hospitals. All subjects were subjected to full history and clinical examination including information on age, sex, BMI, time on HD treatment, the etiology of ESRD, Vascular access, Co-morbidities, evidence of viral hepatitis and medication prescription.
Laboratory tests were done ;Complete blood count (CBC),Iron profile (serum iron, ferritin, total iron binding capacity (TIBC)), Serum creatinine, Calcium, phosphorus, Intact parathyroid hormone (iPTH), Serum Alkaline phosphatase, γ-glutamyl transferase (GGT), Serum Aspartate aminotransferase (AST), Alanine aminotransferase (ALT), Albumin, Total bilirubin, Lipid profile (Total cholesterol, High density lipoprotein cholesterol (HDL), Low density Lipoprotein cholesterol (LDL), Triglycerides (TG)).,CRP (C reactive protein) Titre, HBsAg, HCV Ab, Urea reduction ratio (URR) and KT/V
All subjects were evaluated by Transient elastography Fibroscan® for assessment Controlled attenuation parameter (CAP) measurements to detect liver steatosis grades and liver stiffness measurement (Kpa) value to quantitate liver fibrosis.
There was a significant statistical differences between the HD group I&2 versus control group III as regard mean of BMI, systolic Bp, diastolic BP and mean arterial BP (MABP) which were significantly higher among HD patients studied groups. There was a significant difference regarding mean of serum cholesterol levels, LDL, TG which were significantly higher among HD patients’ groups versus healthy individuals, significantly lower serurm HDL among HD with NAFLD group I versus control healthy volunteers group III.
In the present study although there was non-significant difference between HD patient with NAFLD and HD patients without NAFLD as regards BMI, there was significant positive correlation between CAP value of liver steatosis and BMI among HD patients (r=0.310, P=0.021). There was statistical significant correlation between CAP value and BMI among control group (r= 0.407, 0.026).
