Abstract

The modified SOAR (mSOAR) score is composed of National Institutes of Health Stroke Scale, stroke subtype, Oxfordshire Community Stroke Project classification, age and pre-stroke modified Rankin score and it has previously been shown to be a reliable predictor of mortality and length of hospital stay in acute stroke patients.
Objectives: This aim of this study is to assess the ability of the mSOAR score to predict post-stroke disability measured by mRS at time of discharge from hospital and one month after stroke onset.
Methods: A total of 100 stroke patients ≥18 years were involved in this cohort analytic study. All subjects underwent complete neurologic examination, non-contrast CT brain, the mSOAR score on admission and mRS on discharge and one month after stroke onset.
Results: Fourteen patients (14%) had mSOAR score of 1, 37 patients (37%) had score of 2, 26 patients (26%) had score of 3, 13 patients (13%) had score of 4 and 10 patients (10%) had score of 5. The number of days admitted in studied cases ranged from 3 days to > 30 days with an average of 13.5 days. There was a statistically significant (p = 0.0001) positive correlation between mSOAR score and length of hospital stay. Mortality within 30 days of stroke onset was associated with higher mSOAR score, but this association shows no statistical significance (p = 0.069). The mSOAR score had a high sensitivity and specificity for prediction of moderate to severe disability on discharge and after one month of stroke onset.
Conclusion: The mSOAR score is an excellent and accurate tool for predicting the severity of discharge and one month post-stroke disability.
