Abstract

Cisplatin is widely used to treat many types of cancers such as head and neck and lung cancers. The most common side-effects include nephrotoxicity, neurotoxicity, and ototoxicity.
The mechanisms of Cisplatin nephrotoxicity are complex and involve many processes mainly toxic generation of reactive oxygen species (ROS). Cisplatin also inhibits antioxidant enzymes including Glutathione S-transferase, Glutathione peroxidase, and Super-oxide Dismutase, leading to toxic levels ofROSwithin the cell that leads to endoplasmic reticulum stress and causing cell swelling and necrosis
ALA exhibits significant antioxidant activity protecting against oxidative damage associated with several diseases including diabetes, renal diseases, and aging. It also increases the expression of antioxidant enzymes and participates in the recycling of vitamins C and E.
Coenzyme Q10 (CoQ10), lipid-soluble vitamin-like substance that is synthesized in the body and participates in aerobic cellular respiration. CoQ10 has been shown to be a powerful antioxidant and regulator of mechanisms in the prevention and treatment of cardiovascular disease, diabetes and renal diseases.
This study is an attempt to evaluate the potential synergistic activity of CoQ10and ALA during Cisplatin induced nephrotoxicity.
In this experimental study, rats were divided into 5 groups; group 1; Normal saline control, Group2; Cisplatin control. Group3; Co enzyme Q 10 group plus Cisplatin. group 4; Lipoic acid and Cisplatin. Group5;
Combination group, both Lipoic acid and Co Q10 were given in addition to Cisplatin
The main findings in the study are;
1- The role of antioxidant in preventing renal injuries was confirmed
2- The protective effect of combination therapy is much better when compared with single treatment, we observed significant improvement in creatinine clearance in the group with combined treatment. In addition, there was a significant decrease in the MDA level among combination group, MDA is an important marker for lipid peroxidation and finally, the histopathological findings in the combined group showedthe least kidney damage when compared with each of the monotherapy groups
3- The study had investigated the possible prophylactic effect of ALA and CoQ10, thus the treatment was given for 9 days before the injection and for 3 days after injection. In most of the studies, Cisplatin and the tested agent were given mostly at the same time, however our datain general are comparable to these studies, Adding ALA to CO Q10 in the current study improved the outcomeof our proposed prophylactic course.
