Abstract

The use of articaine 4% and lidocaine 2% local anesthetics during the extraction and pulp therapy of young children under the age of four was examined in a superior randomized controlled trial with two paralleled arms to determine the anesthetic effectiveness and safety for pain and behavior.
In this study, 292 children (146 in each group) between the ages of 36 and 47 months had buccal infiltration injection followed by pulpotomy or extraction of their primary molars. Lidocaine hydrochloride 2% and epinephrine 1:100 000 was given to the control group. Articaine hydrochloride 4% with epinephrine 1:100,000 was used for the intervention, using the (FLACC) and (FBRS) pain measures, child behavior during injection and treatment has been evaluated. Postoperative problems have also been addressed.
There was no difference of statistical significance in the trial’s outcome. In the children’s behavior (FBRS) and (FLACC) pain scales between the two groups before clinical procedures. While children in the articaine group showed significantly positive behavior compared to those injected with lidocaine after finishing the clinical steps (p = 0.035), more children had to intake analgesic postoperatively in the lidocaine group (7.5%) compared to the articaine group (1.4%). The difference was statistically significant (p= 0.011). However, no statistically significant difference was found regarding postoperative soft tissue injury (i.e., lip/check bite) in the two groups.
The influence of both LA agents on children’s blood pressure is average scores. No significant statistical difference was discovered between both the two categories, before LA injection and during clinical procedures. Meanwhile, significant higher systolic and diastolic BP average scores (SBP = 105.73 ± 18.73 and DPB = 63.16 ± 13.43) were detected in the articaine group compared to the lidocaine group (SBP = 98.51 ± 17.39 and DPB = 59.88 ± 11.63), regarding the heart rate (HR),In between two groups pre- and postoperatively, there was no statistically discernible change.
