Abstract

Vitiligo is an acquired pigmentary disorder of the skin and mucous membrane which manifests as white macules and patches due to selective loss of melanocytes .Etiological hypotheses of vitiligo include genetic, immunological, neural, cytotoxic, biochemical, oxidative stress and newer theories of melanocytorrhagy. There are several types of vitiligo which are usually diagnosed clinically and by using a wood’s lamp.
Several treatment options are available for treatment of vitiligo that aims to stop disease progression and/on to attain repigmentation or even depigmentation, but traditional treatments seem to be inadequate.
Phototherapy has been the cornerstone for treatment of vitiligo for years. The combination of topical prostaglandin f2 α with phototherapy has been tried with promising results. Prostaglandin f2 α act through direct effect on melanocyte tyrosinase activity and dendricity and indirectly through FP receptor (prostaglandin F receptor) regulation. The aim of this work was to evaluate the safety& efficacy of intralesional injection of PGF2 α on the outcome of narrow band UVB in treatment of resistant cases of localized stable vitiligo.
In this study, 30 vitiligo patients were treated with intralesional injection of prostaglandin f2 α in combination with NBUVB on one patch and NBUVB alone on another patch.
Methods:
All patients were subjected to:
1. Full history taking. 2. General and dermatological examination. 3. Intralesional injection of prostaglandin f2 α at one patch.
4. NBUVB exposure over both patches on the day following injection twice weekly5. 5. The procedure was repeated weekly for every patient for maximum 3 months (12 sessions).The patients were followed up for 3 months after the last session.
Evaluation of the treatment
1. Photograph 2. Clinical examination. 3. Evaluation of repigmentation was done by three physicians committee. 4. Patient satisfaction.
The results of this study are summarized as following:
The patches treated with prostaglandin f2 α NBUVB showed repigmentation in 93.3% either diffuse repigmentation in 43.3% or follicular repigmentation in 50% of cases, while in the patches treated with NBUVB there was repigmentation in 76.7%.
In the patches treated with combination of prostaglandin f2 α/NBUVB 6.7% of patients showed no improvement, 3.3% showed poor improvement, 26.7% showed good improvement, 43.3% showed very good improvement 20% showed excellent improvement. While with the NBUVB alone patch ,23.3% showed no improvement,46.7% showed poor improvement,20% showed good improvement,10%showed very good improvement and no cases showed excellent improvement. There was statistically significant difference between the two treatment sides. )P<0.001)
With prostaglandin f2 α/NBUVB 6.7% of patients were not satisfied, 6.7% of cases were slightly satisfied, 50% were satisfied and 36.7% were very satisfied. Regarding the side effects there were no systemic complication in any of patients treated with prostaglandin f2 α/NBUVB. 6.7%of patients had agonizing pain during injection, 6.7% had erythema, 6.7% had hyperpigmentation surrounding lesion and 6.7% of patients showed recurrence within 3 months.
