Abstract

Acute coronary syndrome (ACS) is the most common diagnosis at admission to a coronary care unit (CCU). It covers a broad spectrum of patients including those with unstable angina, non-ST-elevation myocardial infarction (NSTEMI), and ST-elevation myocardial infarction (STEMI). As newer and more effective therapeutic options become available, it is equally important to target the right treatment to the right patient. Hence, there is a need for an efficient risk prediction tool to optimise treatment.
As well known, atherosclerosis is an important mechanism in myocardial infarction and the role of inflammation in the process of initiation, progression and plaque de-stabilization in atherosclerosis has been well studied.
White blood cell (WBC) and its sub-type are one of the inflammatory markers that have been studied for a long time in cardiovascular disease. Relative neutrophilia and lymphopenia were a potential condition as a predictor of mortality and recurrence in patients with myocardial infarction.
The ratio of neutrophils and lymphocyte (NLR) is a combination of two independent markers above which is simple to predict the inflammatory state in patients with myocardial infarction as well as predictors of mortality and morbidity.
Global Registry of Acute Coronary Events (GRACE) risk score is a validated scoring system used in patients with acute coronary syndrome (ACS) to predict mortality in the hospital.
Recent guideline Recommends the Global Registry of Acute Coronary Events (GRACE) score for risk stratification and prognosis of patients ACS. The American College of Cardiology Foundation / American Heart Association (ACCF/AHA) guidelines recommends an invasive strategy for patients with high-risk GRACE scores (>140) and conservative approaches for patients with low-risk GRACE score (<140).
We aim to study Neutrophil Lymphocyte Ratio and its relationship to Global Registry of Acute Coronary Events (GRACE) Score in patients admitted with Acute Coronary Syndrome.
This prospective study was conducted on 120 patients with acute coronary syndrome were admitted in CCU in Menoufia university Hospital and Shebin Elkom Teaching hospital, patients were divided into three groups: ST elevation acute myocardial infarction (STEMI) (n=40), Non-ST elevation acute myocardial infarction (NSTEMI) (n=40), and Unstable angina (UA) (n=40).
