Abstract

Background: Endometrial carcinoma is the most common form of gynecologic cancer in developed countries. It is the fourth most common malignant tumor affecting women worldwide.
Aim of the Work: To determine the clinical value and the diagnostic accuracy of 3 dimensional power Doppler angiography (3D-PDA) in pre-operative assessment of cervical invasion in cases of endometrial carcinoma in correlation to the gold-standard post-operative histopathological evaluation. This may provide a basis for preoperative selection of proper treatment options in clinical practice.
Patients and Methods: This study represents a diagnostic accuracy test including 120 women mostly complaining of postmenopausal bleeding. All cases are selected from postmenopausal women attending the outpatient clinic of Gynecology Ain Shams University Maternity Hospital during the period from February 2021 to February 2023. All patients who refuse the study at any time before final histopathological evaluation are not included in statistics.
Results: A positive correlation was found in endometrial volume,Visualization Index and flow index in cervical involvement group compared to non cervical involvement group and a negative correlation between Visualization Flow Index , Bleeding, Hormonal contraception ,Diet, FIGO Stage 1 and myometrial invasion<50%, and there was highly statistically significant difference found between diabetic and hypertensive patient , Histological subtypes and grading(grade 3 & non endometrium type) and Myometrial infiltration >50% between two groups.
Conclusion: Three-dimensional power Doppler angiography is associated with improved accuracy and reliability in the determination and Assessment of Staging of Cervical Invasion in Endometrial Carcinoma. Although our series was relatively small, our data indicate that 3D-PDA analysis of tumor vascularization in endometrial cancer correlates with some prognostic histological characteristics. Further studies in larger series are needed to confirm or refute these preliminary data.
