Abstract

Postpartum family planning is a compelling concern of global significance due to its salience to unplanned pregnancies, and to maternal and infant health in developing countries. Yet, women face the highest level of unmet needs for contraception in the year following a birth. So, there is an important need to study the role of postpartum counselling to increase F.P. use.
The present study was a randomized controlled trial study aimed to assess the effect of the counselling on FP knowledge, attitude, and usage in early 6 weeks post-partum between females who delivered in women’s health hospital, Assiut University. It was conducted from 1st of February 2022 to 1st of March 2022. Follow up was done with phone calls from 15 March 2022 to 15 April 2022.
This study included 120 women who were randomly assigned into: group A with 60 women: who received postpartum counselling about F.P methods and group B with 60 women: who did not receive postpartum counselling about F.P methods. The counselling was done face to face in the postpartum day before hospital discharge by the researcher.
Baseline data were collected from two groups by the researcher using semi structured questionnaire after informed consent was taken. Then intervention group received counselling session and provided with the counselling brochures. Follow up data was collected through phone calls within 6 weeks postpartum.
A pilot study was carried out on 20 women before the starting to collect final data, in order to detecting any
modification needed, and to estimate the required time for implementing the questionnaire which was estimated at 10 minutes.
In Baseline assessment, the questionnaire covered the following items: First personal and socioeconomic data then the menstrual, Obstetric, and contraceptive history. Then, the knowledge about the contraception methods and the attitude about use of contraception methods. The last item was the intention to use any methods after this delivery.
Follow up assessment by filling in a questionnaire which covered the following items: First use of contraception methods, use or not, which method, causes of not using any method. Then the knowledge about the contraception methods and the attitude about the use of contraception methods.
Counselling session: every counselling session was done with intervention group includes Benefits of family planning, addressing myths and misconceptions, personalize counselling and most important information about family planning methods.
The results of this study show that there was a significant difference between the pre and post knowledge score in the intervention group after postpartum counselling with p- value < 0.001, also a significant difference in the post knowledge score between the intervention and control groups at 6 weeks postpartum with also p- value < 0.001.
Also, there was a significant difference between the pre and post attitude score in the intervention group after postpartum counselling with p-value < 0.001 and significant difference in the post attitude score
between the intervention and control groups at 6 weeks postpartum also with p- value < 0.001.
The current study results also show that post-partum counselling had significant effect on use of P.P.F.P. methods within six weeks postpartum, as there were 45 (65.2%) women used F.P. method within 6 weeks postpartum in intervention group and 24 (34.7%) only women used F.P. methods in the control group, with p – value = 0.001.
The most common method used by participants in early postpartum period was Progesterone only pills (POPs). And the most common cause for not using any F.P. methods by participants was ”Desire to have more children”.
Received early postpartum counselling, presence of previous intention to use F.P method and high number of living children increase probability of utilization of F.P. methods.
Healthcare providers especially the family physician, need to be trained to counsel women appropriately about contraception. There is a need for the development of multicenter studies may provide a more accurate results of the effect of post-partum counselling on the knowledge, attitude, and use of F.P. and study the role of the different forms and different timing of counselling should be studied and be compared to postpartum face to face counselling.
