Abstract

Background: Prolonged pressure on the skin may cause frequent injuries to the skin and underlying tissue, known as ischial pressure sores. There are several approaches for prevention, non-surgical and surgical management. In flap surgery for pressure sores, a variety of flap types may be used; each offers certain benefits. However, given the high rates of complications and recurrence, future reconstructive treatments should be taken into account when choosing flaps for reconstructive surgery. This study evaluates the versatility of rhomboid flap technique as a method for ischial sores construction.
Patients and methods: A prospective study was carried out at the Fayoum university hospital’s plastic surgery department. Rhomboid flap technique was applied on twenty patients who had small to medium-sized ischial pressure sores with surface area ranged between 12 to 36 cm2. Proper history was taken, preoperative measures, arterial duplex was done preoperative after flap markings and postoperative follow up and assessment was recorded.
Results: The study group’s mean age was 43.7±13.1, with a range of 19 to 65 years and 80% were males while 20% were female. The main etiology of lesion was paraplegia in 45% of cases. The mean surface area of ulcer was (17.8±6.3) cm2. In 75% of cases, the color match of flap evaluated to be excellent. Similarly, 70% of cases evaluated the flap thickness match as excellent. In 55% of cases, the scar appearance was excellent. Excellent overall satisfaction was reported by 40% of patients, while only 5% reported poor satisfaction. Just 10% of the cases had seroma, and 30% had wound dehiscence of a mean size of 1.33±0.98) cm and required around 6.8±1.9 weeks to heal.
Conclusion: We believe that the rhomboid flap technique is a versatile method for reconstructing small to medium-sized ischial pressure sores due to its technical ease, short operative time, aesthetic outcome, matching skin color and texture, quick recovery time. We think that it is not necessary to search for perforators in case of rhomboid flap reconstruction of small to medium sized ischial pressure sores, but may be of value when reconstruct large sized ischial sores. The main limitations include large defects with large surface area, the lack of a nearby donor site for a rhomboid flap and scars from prior procedures around the ulcer.
