Abstract

This study was done at Sleep unit of Chest Department, Benha University Hospitals in the period starting from February 2018 to February 2019 on 40 patients (the number of males was 30 with percentage 75% and the number of female was 10 with percentage of 25%) with their age ranged from 45 to 77 years with mean age 58.6.
All patients had heart failure diagnosed by echocardiography with LVEF < 45%.
The studied persons were subjected to the following:
1. Full medical history including :
a) History of diseases as chest , hepatic , hypoventilation syndrome , ENT , interstitial lung diseases.
b) Past history : surgery &medications
2. Clinical examination: full clinical examination including general and local chest examination.
3. Plain chest X-ray (Posteroanterior and lateral views).
4. Ventilatory Function Tests (VFT).
5. Laboratory investigations including :
a) Complete blood count (CBC).
b) Liver function tests (LFT)
c) Kidney function tests (KFT).
6. Echocardiography : To diagnose heart failure with LVEF less than 45% , to detect congenital heart diseases.
7. Polysomnography :
Using the device: (SOMNOmedics GmbhAM Sonnenstuhl 63 D-97236 Randersacker Germany) for all patients.
Sleep efficiency, apnea index, hypopnea index, AHI, RDI, baseline SpO2, minimal SpO2, total sleep time, sleep latency, N1 stage, N2 stage, N3 stage REM were measured.
The results of this study showed that :
• Forty patients had congestive heart failure suffer from sleep disordered breathing in the form of OSA with different degrees and CSA.
• 30 males were included represent 75% of studied group and 10 females represent 25% in our study, male sex was predominant in studied group over than females.
• The number of patients had OSA was 25 patients (62.5%) and CSA patients were 15 patients (37.5%).
• There was statistically significance in sex between studied groups as p-value was < 0.05 it reflect that sleep disordered breathing more frequent in males more than females.
• BMI in studied group showed statistically significance between studied group , mean BMI was 30.6% , p-value was < 0.05 , so weight gain , obesity affect the study.
• There was no statistical significant difference in kidney, liver functions in studied group.
• No statistically significance in CXR, FEV1%.
• There was statistical significant difference in LVEF in studied group p-value was < 0.05. It means that patients of CSA had the lower LVEF.
