Abstract This study was performed on 80 subjects divided into 4 groups control, BA with GERB, BA and GERD . Aim: Evaluate the effect of intraesophageal acidification on the airway response in asthmatic patient & also, investigating the role of inflammatory cells & mediators and effect of treatment of GERD on pulmonary functions in BA with GERD. Result: ??Chang in function in Bernstein test. ??Airway remodeling in B.A with GERD and also, in GERD. & asthma. ??Detect predictor of improvement and mechanisms of GERD in bronchial asthma . ??Predictors of asthma improvement were as follows: Decreased frequency of asthma/day, Positive relation of nocturnal asthma to respiratory symptoms, normal chest xray, Grade 1 gastroscopic grading, increase % of lower esophageal sphincter relaxation, BMT, and subbasement membrane lamina propria thickness were if high significant value and can explain why patients did not improve as regard pulmonary function. Recommendation: Treatment of GERD with sufficient the for sufficient duration in patient with BA & reflux. ??Classification of BA patient into BA with GERD and BA without GERD must be considered. ??Antireflux therapy is to be added in the guidelines for treatment of BA with GERD. ??Further research is needed to outline the immunologic and inflammatory relationship between BA and GERD. Also, identifying and verifying predictors of asthma response. |
