Abstract

Two groups of patients were subjected for the study through a retrospective way. The first group included 246 patients with chronic diffuse liver disease aiming at evaluating the laparoscopic yield in diagnosing such cases, in particular for the detection of cirrhosis compared with the microscopic diagnosis as well as the final diagnosis gained from interpretation of all the available clinical as well as investigatory data for every case. The second group included 66 cases among those subjected to both laparoscopic and/or ultrasonographic assessment, within no more than 10 days, for suspected hepatic, peri toneal or GIT malignant lesions. The cases were studied in order to appreciate what laparoscopy can add in the era of modern imaging for such cases ? Our results revealed that: * Combined laparoscopi c / mi croscopi c approach for the assessment of hepatic architecture is more sensitive, more reliable, than a single approach. * Laparoscopic approach alone is reliable when positive for cirrhosis; but absence of macroscopically evident cirrhosis does not rule out its presence. * Histologic detection of cirrhosis has also the same reliability (100%) when positive, but negative results are subject to a high degree of false negativity. * Good sampl ing defini tely increase the y i eld of 1 i ver biopsy.
