Abstract
Osteochondritis dissecans is a process of separation of fragment of subchondral bone with its overlying carti¬lage into a synovial joint. The joint most freqently aff¬ected is the knee. The disease has different etiologies. However, 4 theories are proposed to explain the etiology, exogenous traumatic theory, endogenous traumatic theory, avascular theory and constitutional theory. Exogenous traumatic theory is supported by the following: 1- Similar radiological picture and pathological picture to that of ununited osteochondral fracture. 2- High incidence of traumatic history in patients with osteochondritis dissecans. Endogenous traumatic theory consists of one of the following: 1- Impact of tibial spine against the lateral surface of the medial femoral condyle in full extension. 2- Impact of patella against the classical site with the knee fl exed , 3- Pull of posterior cruciate-ligament on the classical site.
