Abstract

Osteoporosis is one of the most trouhling prohlems for all old females, gynaecologists and orthopedic surgeons. This condition may lead not uncommonly to handicaped old females or even leads to pathological fractures of long hones which may end fatally. So the disease should he kept in mind of all gynecologists for earlY detection and management in the perimenopausal era of a female’s life as its late detection makes the disease very difficult to he controlled. Modern medicine has significantly prolong the human life span and today most women live one third oftheir lives after the menopause which is defined as deficiency disease that is characterized hy lack of estrogen leading to many disorders as osteoporosis, this raises the importance of hormone’ replacement therapy use which gain the popularity nowaday. The aims ’of this study are to identify the regimen which is most suitahle for the prevention of osteoprosis, to study the pattern of hone mineral loss using densitometry in postmenopausal women, and to knoW wheather hormone replacement therapy is a routine or individualized option. This study was performed In fertility Care Unit, Department of Obstetric and Gynaecology, Mansoura University on 73 post menopausal osteoporotic women. These post menopausal women were divided into 3 groups: First: Recieved estrogen and progesterone (n :> I ), Second: Recieved calcium and vitamin D (n ’30), Third: Recieved calcitonin (n 12), Densitometric evaluations were done every 3 months till the em of one year after base line assessment Our study revealed signiflcan difference between the first and the second group as regards the BMI after 3 months, Also at the end of 1st year, there was significan improvement between group 1 and 11 and also between III and 11, So it was apparent that BMD was best on group 1 whileity, worst in group of calcium and vitamin D: Our study also revealed t the longer post menopausal period, the more risk of osteoperosis and less effectiveness of therapy, This fact was shown in our study by significant improvement of bone density status obtained in women than 7 yearS post menopausal in group I and Ill. In our study also we demonstrated that the women which high BMI showed bone mineral density and very appreciable -esportherapy.
