Abstract

Background: Mechanical ventilation can be life sustaining and life saving medical intervention with clear application in a wide variety of illnesses that lead to the common end point of respiratory failure. Aim: In this study, we evaluated planned and unplanned mechanical ventilatory support for patients referred to PSICU and concentrated to evaluate and choose the supposed best available ventilatory mode/modes for the specific type of patients. Result: In this study, the unplanned groups showed increased heart rate, increased CVP, increased FIO2 range, decreased Pao2 and Sao2 , increased Paco2 and decreased pH than planned groups for the basic monitoring and during acute stage with improvement of these parameters during wean and outcome stages. Conculsion: We concluded that when critically ill-patients in PSICU who needed planned or unplanned mechanical ventilatory support managed properly resulted in better outcome which proved by decreased mortality rate (less than 50%) in three groups. However this study proved that planned mechanical ventilation has better outcome than unplanned and this was confirmed by decreased PSICU duration stay, less complications percentage, and less mortality rates of planned groups when compared with unplanned groups.
