Abstract

This study was conducted to evaluate the results of the operative treatment of the comminuted supracondylar intercondylar fractures of the distal end of the femur through the extensile lateral approach utilizing the condylar buttress plate to carefully restore the articular surface, anatomically. This study included twenty-two patients with a mean age of 39.8 years presented to Mansoura Emergency hospital with either simple or open complex fractures of the distal end of the femur treated operatively with extensile lateral approach through which the extensor mechanism of the knee was elevated with a block of bone removed from the tibial tuberosity. There were 8 female and 14 male patients with 12 patients were injured by falling on stairs on a flexed knee, while the other 10 patients were injured in motor vehicle accident. At follow-up period of 18 months, the results were evaluated according to the functional evaluation of Sanders et al, 1991. It was found that 81.8% of the patients had favorable results, while only 18.2% had unfavorable results. There were no delayed union or lion-union in our study. The major complication in this study was deep wound infection occurring in 2 patients who had delayed open reduction and internal fixation of grade II open supracondylar intercondylar fractures of the distal end of the femur.
