Abstract Management of renal stones has been always a challange to the urologists. The objective of stone management is not only stone clearance but most important preservation of renal function with the least morbidity. The issue of renal parenchymal damage after nephrolithotomy whether open or percutaneous is still controversial in the literature. Between August 2001 and February 2002, 78 patients with renal stones who fulfilled our inclusion criteria were included in the study, 38 patients were treated by PCNL, 24 patients were treated by open nephrolithotomy and during the same period 16 patients were treated by pyelolithotomy and were included as a control group. Renal function and morphology were assessed preoperatively, 1 week, 3 months and 6 months postoperatively using scintigraphic studies. There was no change in postoperative renal function in renal units subjected to PCNL via one track and those subjected to PCNL via more than one track. The group of patients subjected to open nephrolithotomy, the type ischemia whether warm or cold was found to have no impact on renal function so long as it was within tolerable limits. There was significant deterioration in renal function and development of pyelonephritic changes in the renal units subjected to open nephrolithotomy while the renal units subjected to PCNL showed stable or mild degree of change in renal function postoperatively with nearly no morphological changes. |
