Abstract

Background: While the incidence of breast cancer as well as the recovery rate continues to rise, breast cancer is hardly a new affliction. Incidents of breast cancer have been documented back to the early Egyptians when the popular treatment was cautery of the diseased tissue. Surgery was practised but it was an extremely radical treatment considering there was no anesthesia or antisepsis available. Aim of Work: To evaluate breast preservation surgery as regard: Local recurrence detection during follow up period and if present correlate its presence with several variables; Cosmetic results in the follow up period using 4­point scoring system; Surgical accessibility of the tumor in different sites, sizes and incisions; and To predict bone marrow micrometastasis and correlate its presence with several variables. Patients and methods: This prospective study of 30 patients was carried out in Mansoura university hospital, department of general surgery between July 2000 and July 2004. The age of the patients ranged from 30 to 60 years with the mean of 45 years. Follow up period was between 12 to 48 months. All patients were diagnosed by true­cut needle biopsy. Results: from that point we conduct our study on 30 patients with early stage breast cancer fulfilling, the criteria for breast conservative therapy pulling in mind four things, local recurrence, cosmetic results, surgical accessibility by using different approaches and surgical techniques to perform lumpectomy and axillary evacuation, and evaluation of bone marrow micrometastasis. All patients receiving radiotherapy and all patients received adjuvant chemotherapy. All patients with positive or unknown hormonal receptors (ER +ve or PgR +ve) received hormonal therapy, and we follow tip our patients for 4 years. Conclusions: At the end of our study we conclude that local recurrence is not significantly correlated with the variables in the study and we need longer period of follow up and more number of patients. Cosmetic results were significantly correlated with the tumor size and the type of incision with excellent cosmoses in tumors less than 3 cm in the upper outer quadrant treated by axillary incision.
