Abstract

Introduction: Polytrauma can be defined as Injury to several physical regions or organ systems, where at least one injury or the combination of several injuries is life threatening with the severity of injury being > 16 on the scale of the Injury Severity Score (ISS). To be differentiated from multiple injuries, which is not life threatening, or a severe, life-threatening single injury (barytrauma). Aim of work: To determine the independent predictors of adverse outcome in polytraumatized patients. Patients and methods: This retrospective study was conducted on 1311 polytraumatized patients admitted to Mansoura Emergency Hospital, over a period of 60 month from January 2000 to January 2005, with the aim to detect independent predictors of outcome in polytraumatized patients using patient history, trauma history, vital signs, laporatory investigations, scoring systems, and type of management. Mortality was our outcome predictor. Results: These nine factors were subjected to multivariate logistic regression analysis and to recognize independent predictors of outcome in polytraumatized patients. Independent predictors were GCS, endotracheal intubation, ICU admission and injury severity score (ISS). This study has some limitations; it focused on severely polytramatized patients only (ISS ≥ 16). Also, it excluded penetrating trauma and burned polytraumatized patients. Conclusions: This study may comprise a start for many future researches that consider all types of trauma, and other outcome measures such as morbidities and length of hospital stay. And also for more advanced prospective studies for long term evaluation of quality of life of these patients.
