Abstract The aim of this was to evaluate the possible role of GLP-1 in the progressive deterioration of diabetic control observed in the majority of long standing dibetices and to compare different lines of treatment as regards beta cell function and GL-1 secretion and find out if disturbance in GLP-1 secretion antedates the occurrence of frank diabetes. The results of this study showed that there was a decline in GLP-1 level in old diabetics thus providing a factor that may explain the deterioration of diabetic control in type 2 diabetes. Also, a-glucosidase inhibitors can mobilize the endogenous pool of GLP-1 more than other modalities of treatment although on clinical basis, they have mild to moderate effect on glycaemic control when used alone and they meny have a significant synergistic value to the other antidiabetices. Finally, GLP-1 level may increase in the relatives of diabetic patients thus, it may help to anticipate the occurrence of diabetes in them. |
