Abstract

Studies have shown that subantimicrobial dose of doxycycline (SDD) results in statistically and clinically significant improvements over those achieved by scaling and root planing alone when it is used as an adjuvant to scaling and root planing in treatment of periodontitis. Furthermore, it has been reported that, CoQ10 improves periodontal conditions when it was used systemically or topically.The present study was designed to clinically examine and compare the therapeutic effectiveness of systemic administration of either SDD, CoQ10 or combination of both as an adjuvant therapy to scaling and root planing to the effectiveness of scaling and root planing alone in treatment of chronic periodontitis patients. Also, it was designed to evaluate the effect of such different treatment modalities upon the crevicular osteocalcin level.A total of 40 patients with chronic periodontitis were included in this study. They were randomly distributed into four groups of ten patients. Group I: Was treated by scaling and root planing in addition to 20 mg doxycycline taken orally twice daily for three months. Group II: Patients were treated by scaling and root planing in addition to 30 mg CoQ10 twice daily for three months. Group III: Patients were treated by scaling and root planing combined with the same doses of the previous two drugs for three months. Group IV: Patients were treated by scaling and root planing alone and considered the control group in the study. Clinical assessment was performed using GI, PI,BI, PPD and CAL. Clinical parameters were recorded for all the patients at baseline time, after one and three months of treatment. Moreover, GCF samples were collected from all the patients at the same periods for assessment of osteocalcin level by electrochemiluminescence immunoassay in order to evaluate the effect of each treatment modality on GCF osteocalcin level. Clinical results revealed that, mean values of almost clinical parameters decreased significantly after one and three months when compared to their corresponding mean values at baseline time. Also, these parameters decreased significantly after treatment for three months when compared to their corresponding after treatment for one month. . Comparing the four groups together, showed that significant changes were found between the mean values of the clinical parameters at the same time but it does not follow a consistent pattern favoring specific group. However, mean values of GCF osteocalcin level did not decrease significantly after one and three months of treatment on comparison to each other and with their corresponding mean values at baseline time. The present study demonstrated that administration of either SDD or CoQ10 twice daily for three months as an adjuvant to scaling and root planing resulted in more significant improvement in clinical parameters than that achieved by scaling and root planing alone. Furthermore, treatment of chronic periodontitis by combination of both drugs for three months showed better therapeutic effect than that of each drug alone.
