Abstract

-Sernx protein-bound carbohydrete levels were shown to be elevated in patients with diabetes mellitus. The demonstration of carbohydrate staining material in the retinel and renal lesions of diabetics raised the possibility thet abnormal metabolism of czrbohydrate containing proteins may be of etiologic importance in the development of diabetic aagiopathy. For this purpose a. total number of 91 cases with age ranging from 10 years to 60 years were carefully selected from amongst the attendants of the out patient clinic of medicine in Tanta University Hospital. These included 20 adult onset diabetics without microangiopathy, 20 adult onset diabetics with microangiopathy, 23 juvenile diabetics and 28 apparently healthy subjects as reference group ., All cases were subjected to careful clinical and laboratory investigations to assess the diagnosis. The study was conducted according to the following scheme : A) 1-Estimation of total serum protein by Biuret method. 2-Sem protein fractionation by cellulose acetate zom electrophoresis, 3 - Imunoelectrophoresis f o r glycoproteins including alpha-1-scidglycoprotein, alpha-1-antitrypsin, alpha-2-macroglobulin,haptoglobin, oeruloplasmin and transf errin, B) The next step was t o c o r r e l a t e -he changes i n the p r o t e i n p a t t e r n and those occuring i n the d i f f e r e n t individual glycoproteins in d i f f e r e n t diabetic groups, adult onset w i t h the without microangiopathy, juveniles as compared with t h e r e f e r e n c e group, C ) A trial was made to work out a s p e c i a l imunochemical p r o f i l e f o r microangiopathy corfiplicated diabetes to be d i s t i n c t from the adult onset d i a b e t i c s tvlthout compl i c a t i o n s , In the light of the r e s u l t s which were fully discussed the following points seemed to be of p a r t i c u l a r i n t e r e s t : 1 - Serwn alpha-X-acidglycoproteins were found to be increased i n all d i a b e t i c groups especialy those w i t h microangiopathy. 2 - Semx haptoglobin showed increased concentration i n a l l d i a b e t i c groups and more prounounced i n cases with microangiopathy. 3 - Serum alpha-2-macroglobulin increased in a l l diabetic groups with a prominant increase i n those with micro4 Alpha-1-antitrypsin was increased i n all diabetic groups, but there is no significant difference betmeen adult onset with and without microangiopathy, 5 - Cerulo~lasmin showed mild t o inoderate elevation in adult onset, juvenile ad casea with microangiopathy. 6 - Transferrin levels were lower i n all diabetic groups when compared with reference groups.
