Abstract

Mortality and long term neurologic sequelae are still frequent complications of acute meningitis. The outcome of the disease depends mainly on the early diagnosis and prompt treatment. The aim of the present stucfy is to test the reliability of CSF-ferritin in early diagnosis of acute meningitis and the differentiation between septic aseptic meningitis. For this purpose, 90 patients were selected from inpatient department of Menouf Fever Hospital with clinical presentations of suspected acute meningitis. In addition to 30 persons (controls) were clinically free of symptoms and signs of meningeal irritations undergoing spinal anaethesia for various non CNS surgical indications during the period from January, 2004 to November, 2004.
Patients and controls were classified into the following groups; group I: comprised 90 patients with clinically suspect acute meningitis. According to the results of CSF examination, they were classified into 3 subgroups: Group la: comprised 30 patients with septic meningitis. Group Ib: comprised 30 patients with aseptic meningitis. Group Ic: comprised 30 patients with suspected septic meningitis. Group II: comprised 30 persons undergoing spinal anaethesia for various non CNS surgical indications as control group.
The patients and controls were subjected to the following: (1) Complete history taking and full clinical examination with stress on signs and symptoms of meningeal irritations. (2) CBC. (3) Measurement of serum ferritin. (4) LP and CSF examination including: [a] aspect and tension of CSF. [b] CSF total and differentional leucocyte count, [c] CSF-
