Abstract

The results of this study demonstrate that submandibular gland excision is a highly successful procedure with minimal morbidity, which is particularly important as most patients undergo surgery for non-neoplastic disease. The most important complication is damage to the mandibular branches of the facial nerve. This is due to anatomical variation in the course of this branch of the facial nerve, inappropriate surgical technique, and the small caliber of the nerves involved. It is believed that it is more dangerous to identify this nerve deliberately than to have a method of excision that will safeguard the nerve in the majority of cases. The study highlights the reliability of the low, ”non-identification” approach in submandibular gland surgery. It has been shown that this method of removal of the submandibular salivary gland gives maximal safety to the mandibular branches of the facial nerve, even when the gland has been involved in a chronic inflammatory process secondary to calculus obstruction where adherence to surrounding structures is seen frequently.
