I3ilc duct injuries are very uncommon but constitute a serious ami poicnlially devastating complications of both open and IX’. They am devastate tin individual by turning him into a ”biliary cripple” and most ultimately die of hepatic failure. They often result from errors of human judgement and are thus preventable,
latrogenic bile duct injuries represent a challenge even for Ihe most skilled biliary surgeon. The advent and widespread use of lapnroscopic choleeystcciomy has dramatically increased the incidence of bile duel injuries and strictures associated with cholecyslcetomy.
Early recognition of injures, mature clinical judgement and experienced repair of this injury is of utmost importance to the good
prognosis of the patient. The best chance fora successful repair is the first chance.
The diagnosis ofiatrogenic biliary injuries is first nude or suspected oil clinical basis, Ihcn a number of laboratory iesls and imaging techniques arc used to confirm and define precisely the nature of the injury.
Advances in endoscopic and radiological technology have suggested wider use of non-operative options as alternative means of relieving biliary obstruction, offering an alternative to technically difficult and occasionally unsuccessful surgical procedures.
Surgical repair of biliary strictures is best achieved with a biliary enteric anastomosis; Roux-en-Y hcpalicojcjunoslomy carried out near the
