Abstract

It has been recognized that medical care has the potential to cause harm. This ham. may result from system failures. Thus to ensure patient safety, which is an integral part of the delivery of quality care, a comprehensive patient safety system should be developed. This system includes both, a culture of safety and an organizational support for safety processes. A culture of safety is a culture that encourages healthcare providers to identify potential or actual errors, take appropriate actions to prevent harm, and disclose appropriate information on errors to facilitate learning and the redesign of care processes. This should take place through a non punitive environment which realizes the limitations of health care professionals and inevitability of error. There is growing awareness that many patients are harmed in Egyptian hospitals when receiving healthcare. However, the awareness that this harm may be due to health system failures, and not just failures of direct healthcare providers, is slower to emerge. In , addition, the healthcare sector in Egypt does not identify and collect information on errors to know their incidence and prevent their recurrence. Thus there were no relevant Egyptian publications assessing the culture in hospitals, or identifying the incidence and causes of errors. This study aims to: Assess the perception of physicians and managers about the frequency, types and causes of medical errors. Assess personal experience of physicians and managers with medical errors in terms of committing or witnessing an errorDetermine the views of physicians and managers towards. reporting of medical error and adverse events, and barriers to, reporting Assess the extent to which safety,’ culture dimensions are implemented in the hospital The study was conducted at three general hospitals in Alexandria, one representing Health Insurance Organization hospitals (Gamal Abd El Nasser Hospital), the other representing Ministry of Health and Population hospitals (Abou Kir Hospital), and the third representing University hospitals (Medical Resaerch Institute Hospital). It is a descriptive study, cross sectional in design. The study sample consists of all physicians and managers in. study hospitals. In Gamal Abd £1 Nasser Hospital 129 physicians responded from a total of 210 physicians, while in both Abou Kir, and Medical Resaerch Institute Hospitals a total of 94 physicians responded from a total of 132 and 142 respectively.’ A total of nine managerS representing, all managers at study hospitals responded. As regards data collection, self administered questionnaire was administered to physicians and managers. The questionnaire included four main sections The first section includes demographic and professional characteristics of physicians. The second section includes 44 statements for physicians and 42 statements for managers to assess the culture of safety in their hospitals.
