Abstract

Adequate dental health for everyone has been the con¬ sistent goal of the dentists individually and through their professional societies. The soldiers of the Oentral Security in Alexandr ia are adolescent recruits who spend three years in a closed community, where no orc;anized system for dental care can be found. Therefore this study was carried out , Rural soldiers had lOwer caries prevalence than urban. Prevalence of caries increased by age and consequently by increased duration of recrui tment. Soldiers in El Max zone which includes the initial training centre, where the newly recruited soldiers are found, had the lowest mean of caries index. Oral b.yp.;iene pattern El Max zone which provided dental health educat~on had the lowest mean of oral hygiene index. The oral hygiene index significantly increasedwith increase in age. Periodontal condition Is associated with oral hygiene condition, therefore the better periodontal index was among soldiers of El Max zone than the other two zones. The periodontal index improved during recrui tment . Treatment needs as assessed by examination In the present study only 13 from the total sample did not need any kind of tre~tment, filling need was 53.32 from the sample most of them for one surface or two surfaces. Periodontal treatment need was 75.37, most of them in the form of propr~laxis and oral hygiene instruction. Condition5 needing i~~ediate attention constituted 15.b4 most of them pulpally involved and necrotizing ulcerative gingivitis. Prosthodontic needs constituted 22.91. The present study revealed that ’70 of those who were clinically assessed as needing treatment felt personally that tlillY required it. Only 24.62 of the sample had actually trieu to obtain treatment. Utilization The main reason for las’t dental visi t was toothache constituting 81.73 and the main source for treatment was general hospital and the ~ental clinic in El Raas El Soda zone. The most common barrier for utilization in this study was financial barrier constituting 26.90, which was more for the zones without clinic. No time for treatment, unsa¬ tisfactory previous visit and fear were other barriers for . ( seeking dental care. The most motivatinc factors were, financial followed by the behaviour of individual himself .i (according to soldiers Op\nion) , accessability of clinic and presence of pain. With respect to the preferred clinic to wh’.,h the soldiers would go if the need arise, most of the soldiers in El Raas El Soda preferred to go to a clinic in the zone(54.l7). The main reasons stated for this preference was because of accessibility (5b.46) and for free care (21.53) and for free care (21.53 perferred private clinics, approximately all of them stating that this was for better care. Those who preferred general hospital clinic (41.50), their reason being for safer treatment. Of the restorative needs ’b8.l7 was unmet )from extrac¬ tive needs 34.06 was unmet and}from replacement needs 9B.42was unme t . Every soldier was estimated to require on the average 1.35 hours to cover his treatment needs. In an attempt to calculate the number of dentists needed by each zone it was revealed that, El Raas El Soda and El Max each require two dentist and El Dekheila requires five dentist to cover this need. The most important recommend revealed through this study¬ includes comprehensive dental programme which provided through: 1. Initial care This includes scomprehensive examination of all recruits in the three zones to discover and treat the cases before reaching the emergency stage. 2. Annual.care Annual examination for the new recruits to detect and correct the ne’w dental diseases. 3. Preventive measure This may be provided by some form of health education performed periodically to clarify the importance of dental treatment. a. In the zones without <:linic the main barriers were financial and no time t’or treatment outside the zone. Therefore it is recommended that a dental clinic should be available and accessible to serve the soldiers in both zones. b. In the zone with a clinic the main barriers were fear and unsatisfactory previous visit. Therefore it is recommen¬ ded that, the dentist should show patience and underst~lding with the soldiers.
