Abstract The latter represents an early stage of diabetic renal disease, which can progress to clinically overt nephropathy. In IDDM many studies referred to certain relationship between raised albumin excretion rate (AER) and subsequent development of clinical nephropathy. The suggestion referred here to microalbuminuria as a true sign of early disease rather than of susceptibility marker. In NIDDM, certain studies related AER to later development of clinical proteinuria and increased mortality especially from cardiovascular disease. Consequently, MA represents here an expression of incipient general disease of blood vessels supplying the kidney as well as the heart. Increased cardiovascular morbidity and mortality among microalbuminuric diabetics (Type II) may not be primarily attributed to hyperinsuliemia or insulin resistance. Certain predictors for MA were reported before like increased level of platelet activating factor which may serve as sensitive indicator of renal injury in diabetics. Hypertension can, also, increase the risk of cardiovascular disease for diabetes and MA development |
