Abstract

The management of carotid artery disease has been subjected to more scientific scrutiny than any other vascular pathology. Despite a vast evidence base, however, the management of carotid artery disease continues to arouse much controversy and polarized debate.
Carotid endarterectomy (CEA) stands as an approved method for revascularization of symptomatic and asymptomatic patients with extracranial carotid occlusive disease. Enthusiasm has grown for carotid artery stenting (CAS) as a less invasive procedure, recommended in specialized subsets of patients, such as restenosis after CEA, radiation-induced carotid stenosis, anatomically high lesions and in higher-risk patients. Nevertheless, its use in lower risk patients has not yet been established.
In our study, 19 patients were enrolled and underwent 20 CAS procedures. All the patients were symptomatic. Symptoms were mainly transient ischemic attacks and strokes. The risk factors were identified. Stenotic lesions ranged from 60% to 99% distributed according to etiology.
The technical success rate was 100% (20/20). The protection devices were used in all cases and all were of the filter type. No minor or major strokes developed during our procedures. Only 2 procedural TIAs were observed, each resolved spontaneously within minutes of its onset and without residual deficits. No procedural death was reported. Contrast agent-induced renal failure did not occur in any patient.
