NHLs are heterogenous group of lympho proliferative malignant diseases have differing patterns of behavior and response to treatment.Combination chemotherapy may be treatment of choice for advanced stage aggressive NHLs. The newer generations of it depend upon using more than one active drug, high dose intensity,
administration of these drugs over short period of time (dose density) and early exposure to non-cross resistant drugs.
Patients with 2 or more risk factor have less than a 50% chance of relapse-free and overall survival at 5 years.
This study had been carried out in Medical Oncology and Haematology Unit Internal Medicine Department, Zagazig University hospitaL Between April 2002 and May 2004 on 40 patients with
Denovo aggressive NHL with international prognostic index (IPI) >= 3
All patient were subjected to complete history, complete physical examination, staging ”according to Ann Arbor staging system” and routine laboratory investigations.
All patient were treated with AIT regimen and evaluated for response after 3 cycles and responding patients continued for 9 cycles then response evaluated and toxicity according to WHOtoxicity scale.
Disease free survival and overall survival at 1 year were estimated.
Our result showed that:
ATT protocol induce CR in 72.5% ofpatients, PR in 22.5% of patients (OR 95%) and NR in 5% ofpatients.
High LDH, poor performance status and number of extranodal sites were statically significant in predicting poor response.
Toxicity to ATT protocol was accepted except renal toxicity which lead to death in one case.
I year disease free survival (DFS) was 52% and overall suevival was 67%.
