Abstract

Insulin resistance is not simply a problem of deficient glucose uptake in response to insulin, but a multifaceted syndrome that increases significantly the risk for cardiovascular diseases. Several studies have shown increased ox-LDL levels in insulin resistant patients suggesting that the protecting role of HDL-C against oxidative modifications of lipoproteins is impaired. Paraoxonase is one of the enzymes located on HDL-C particle and it reduces the accumulation of f- lipid peroxides on LDL-C. Therefore, we designed this work to study changes of serum paraoxonase activity in insulin resistant states namely [patients with type2 diabetes mellitus, obese first degree relatives of diabetic patients and hypertensive patients] and try to find out any relationship between these changes in paraoxonase activity and each of the degree of insulin resistance and the degree of atherosclerosis in these groups of patients.
Sixty subjects were included in this study, they were divided into two main groups:
Group .4: Included 15 healthy subjects as control group.
Group B: Included 45 insulin resistant patients, they were categorised as insulin resistant by HOMA index more than 1.64, they were divided into three subgroups according to the cause of insulin resistance:
GroupB1: Included 15 patients with type 2 diabetes mellitus, they were normotensive and non obese.
GroupB2: Included 15 obese first degree relatives of diabetic patients, they were normotensive and non diabetic.
Group B3: Included 15 known hypertensive patients with or without rHD , they were non diabetic and non obese.
