Abstract

The most important factor that influences therapeutic outcome of cancer larynx in the development of metastasis of cervical lymph nodes.

The prevalence of cervical lymph node metastsis in patients having cancer larynx without enlarged cervical lymph nodes is (31%).

The prevalence is affected by number of pathological parameters including perineural vascular and muscle invasion and the incidence of metastasis increases with increasing the number of pathological parameters involved into the tumor.

Elective neck dissection is still debatable issue, but most studies point to the use of selective neck dissection to attack the first echelon lymph nodes level II, HI and IV with limited removal of non lymphatic structure in the neck where there is no lymph nodes enlargement.

The aims of this study are:

1 To describe the prevalence of occult metastasis in the clinically negative cervical lymph nodes (NO) in patients with cancer larynx

2 to demonstrate the correlation between pathological parameters of the tumor (size, stage, invasion of the cartilage, muscles, nerves and blood vessels) and the presence of occult metastasis

3 To describe the role of elective neck dissection as a prophylactic measure.
