Abstract

In Egypt, colorectal cancer is common and represents only three percent of all malignancies{excluding tumors of the nervous system) (Sherif m 1997) lt is the third most common tumor in males after urinary bladder and Iymphohemopoietic malignancies , and in females it ranks fifth after breast ,Iymphohemopoietic ,cervical, and urinary bladder cancer (Mokhtar N 1991) The recent progress of molecular biology had revealed several genetic abnormalities, which are related to the behavior of this disease .the identification of these markers may help the . prediction of certain group of patients who may need other form of treatment the detection of several ontogeny’s as P53,BcI-2,Bax,k-ras,C-myc had been recently correlated to the progress of disease.
The aim of the current study is to evaluate the prognostic significance of molecular markers as P53, bcl2 and Bax as well as the proliferation marker AgNOR in colorectal cancer. Furthermore, it is designed to discuss the prognostic value of clinicopathological parameters of the tumor. The present study was performed on 31 patients with colorectal carcinoma (CRC); those were submitted to radical colonectomy surgery. From each block Hematoxyline and Eosin (H&E) stained slides were prepared and examined. Immunohistochemical staining for oncogene products and argyrophilic nucleolar organizer region (AgNOR) method stains NOR-associated proteins (Howell WM et aI., 1980) were performed in Department of Pathology, NCI, Cairo University, EYGPT. Patients’ outcome was determined from the hospital and physicians office charts. Median follow up was 18 months. The statistical analysis was done using computer statistical program, SPSS 9 Statistical analysis was done according to Ingelfinger et al(1994) and Knapp and Miller (1992) The present study included 31 patients with colorectal cancer. With median age of 52.
Bleeding was the main symptom at presentation (51.5% pathological examination of the tumor specimens revealed that 67.7% were adenocarcinoma, 38.7% in the left colon, and 22.5% were gradelll,48.4%of the tumors were in Duke’s stage C ,Lymph node were positive for metastasis 35.5%of patients and 25.8% of patients ’presented with metastasis (Table 2), 32.2% of cases were positive for P53 immnuostaining , 54.1 % of the tumors.
