Abstract

Rheumatic fever still forms a major health problem in Egypt and other developing nations. Its importance arises from the fact that it can result in chronic heart disease especially among the poor and medically deprived children. The main objective of this research is to study the school health services provided to school students regarding primary, secondary and tertiary prevention of rheumatic fever and rheumatic heart four subobjectivies were outlined: I, To assess the services provided through the comprehensive physical examination for early detection of rheumatic fever and rheumatic heart disease. 2. To study the services provided by G.Ps in the school clinics and specialists in polyclinics for rheumatic patients as well as to illustrate their opinion regarding methods for improvement of existing services. 3. To assess the quality of care through satisfaction of rheumatic patients regarding services provided. Data were collected through observation of G.Ps working at the chosen sample of preparatory schools while performing the comprehensive examination to check their activities in connection with early detection of rheumatic fever and rheumatic heart disease as well as checking the examination place, presence or absence of certain required equipments and supplies. Moreover, all G.Ps working in school clinics present in all school levels in Alexandria and all specialists [Cardiologists, Internal Medicine and pediatricians] working in all comprehensive Health Insurance Clinics were interviewed to determine their opinion regarding existing services provided for the prevention and treatment of rheumatic fever and rheumatic heart disease as well as their actual role in such services and their recommendations for improvement of services. In addition rheumatic students attending comprehensive Health Insurance clinics were interviewed to measure the quality of care through their satisfaction regarding services provided in schools, comprehensive Health Insurance Clinics as well as hospital services if there was previous admission. Analysis of the collected data revealed the following results: School clinics were available in 85.71 of schools and were found to be satisfactorily quiet in 78.57 of schools. The examination bed was available in 85.71 of schools while tongue depressor and thermometers available 78.57 of schools while were in sphygmomanometer was available in only one school(7 .14). It was observed during the comprehensive examination that performance of history taking and physical examination for students (concerning rheumatic fever and rheumatic heart disease) was defective in all districts.
